Job summary

UCLA Health in Los Angeles is seeking a Nurse Care Coordinator within the Brain Tumor Program to provide care coordination, follow-up, and patient education for brain tumor patients and their families.

You will be the main point of contact, coordinating care among patients, families, and the Multidisciplinary Cancer Care Team, guiding patients through their cancer care journey and ensuring smooth transitions across services.

Qualifications

• CA RN License
• BLS from the American Heart Association or American Red Cross
• BSN or MSN required for all new hires effective 1/1/2015, current UCLA Health employees may be exempt.
• CNII: Greater than 6 months nursing experience
• Ability to complete a Nursing physical assessment
• Skill in developing a plan of care including setting goals, monitoring and documenting progress
• Ability to implement interventions that are pertinent to goals established in the plan of care
• Ability to evaluate the effective of interventions toward the attainment of established goals and to recognize the need to reassess or reset goal parameters
• Ability to delegate tasks in a management style that is consistent with promoting leadership and teamwork
• Knowledge base to provide education to patients, families, and other staff regarding clinical and patient care issues
• 2 years in neurology/neurosurgery

Responsibilities

• Provide care coordination, continuity of care, follow-up, support, and education for Brain Tumor Program patients and families.
• Serve as the main point of contact for all Brain Tumor Program patients.
• Coordinate care between patients, their families, and the Multidisciplinary Cancer Care Team.
• Guide brain tumor patients through their cancer care experience.
