University of California in Los Angeles (UCLA Health) Brain Tumor Program seeks a Nurse Care Coordinator to lead care coordination, follow-up, and patient education for brain tumor patients and families. You will act as the primary contact, guiding patients through multidisciplinary cancer care and coordinating with clinicians to ensure seamless care experiences.

The role emphasizes care planning, nursing assessments, and collaboration with neurology/neurosurgery teams to optimize outcomes

Competenze

• CA RN License required.
• BLS from AHA or Red Cross.
• BSN or MSN required for all new hires effective 1/1/2015; UCLA Health employees may be exempt.
• CNII: Greater than 6 months nursing experience.
• Ability to complete a Nursing physical assessment.
• Ability to develop a plan of care with goals, monitoring and progress.
• Ability to implement interventions aligned with care goals.
• Ability to evaluate effectiveness toward goals and reassessment.
• Ability to delegate tasks in a management style promoting leadership.
• Knowledge to educate patients, families, and staff on clinical care issues.
• 2 years in neurology/neurosurgery.

Description

Under the direction of the Directors of the Brain Tumor Program (BTP), also known as the Brain Tumor Integrated Practice Unit, you will provide care coordination, continuity of care, follow-up, support, and education for patients of the Brain Tumor Program and their families. In this role, you will serve as the main point of contact for all patients in the Brain Tumor Program and will be primarily responsible for coordinating carebetweenpatients,theirfamilies, and theMultidisciplinaryCancerCareTeam. You will also help guide brain tumor patients through their cancer care experience.

Hourly Salary Range $65.76- $97.76.

Qualifications

• CA RN License
• BLS from the American Heart Association or American Red
  Cross
• BSN or MSN required for all new hires effective 1/1/2015,
  current UCLA Health employees may be exempt.
• CNII: Greater than 6 months nursing experience
• Ability to complete a Nursing physical assessment
• Skill in developing a plan of care including setting goals,
  monitoring and documenting progress
• Ability to implement interventions that are pertinent to
  goals established in the plan of care
• Ability to evaluate the effective of interventions toward
  the attainment of established goals and to recognize the need to reassess or
  reset goal parameters
• Ability to delegate tasks in a management style that is
  consistent with promoting leadership &   &nbsp
• Knowledge base to provide education to patients, families,
  and other staff regarding clinical and patient care issues
