UofL Health - Brown Cancer Center is seeking a Nurse Navigator to advocate, educate, and coordinate care for oncology patients and their families. This role operates within a multidisciplinary team across the ULH system to guide patients through treatment pathways and connect them with resources.

The position emphasizes patient education, care coordination, and effective communication, with travel to multiple ULH locations as needed. Oncology experience and an RN license are required.

Formación

• Graduation from an accredited nursing program.
• Associate Nursing Degree required; BSN within 2 years of hire.
• Current RN license in Kentucky.
• 2+ years oncology bedside experience.
• BLS certification from American Heart Association.
• Ability to work autonomously.

Responsabilidades

• Educates patients and families about disease, treatments, and side effects.
• Coordinate patients through the continuum of care with the multidisciplinary team.
• Serve as liaison between disciplines and supportive/community services.
• Coordinate weekly multidisciplinary conferences and track treatment outcomes.
• Compile data, monitor outcomes, and suggest process improvements.
• Provide referrals and support for follow-up care.
• Travel to multiple ULH locations as needed.
