Qualifications

• Typical Requirements: One to three months experience in a human services setting.
• Experience working with individuals with brain injury preferred.
• High school diploma or equivalent required.

Responsibilities

• Develop, implement, and document individual treatment support plans.
• Perform rehabilitative interventions, behavioral monitoring, and schedule appointments.
• Provide transportation and assist community integration.
• Assess physical and emotional status; report changes to supervisor.
• Ensure site safety; oversee daily living tasks and housekeeping.
• Record documentation: logs, timesheets, and forms.
• Carry out crisis intervention when needed.
• Maintain contact information with Relatives and HR staff.
