Brain Injury Association of Colorado (BIAC) seeks a Brain Injury Reentry Specialist to work face-to-face with clients transitioning out of the legal system in Denver. You will assess brain injury symptoms and support reintegration into the community.

Responsibilities include managing a caseload of 25-35 survivors from 1-2 courts, screening 100 individuals annually, developing wraparound re-entry and symptom plans, facilitating weekly psychoeducation groups, and coordinating with partners to link

Formación

• Bachelor’s degree or equivalent experience in a related field.
• Experience navigating systems in human service, case management, social work, legal systems, or other applicable settings.
• Experience with small group facilitation.
• Travel to other meetings and events within the state, sometimes after hours and on weekends.
• Bilingual English and Spanish – preferred.
• Experience with individuals who have cognitive disabilities and/or behavioral health challenges, and involvement in the legal system.
• Experience teaching life skills, including organization, financial management, healthy lifestyle, social skills, job skills, and computer and internet use.
• Knowledge and experience applying person-centered and culturally competent practices in interactions with clients, co-workers, professionals, and other community members.
• Experience using a database tracking system (Salesforce preferred).

Responsabilidades

• Manage caseload of BI survivors from 1-2 courts; 25-35 survivors.
• Screen at least 100 legal system-involved individuals for brain injury annually.
• Develop comprehensive re-entry plans with at least 100 survivors annually.
• Develop symptom support plans and wraparound service plans for 100 survivors annually.
• Facilitate weekly AHEAD psychoeducation groups.
• Provide wraparound services to stabilize clients during reintegration.
• Coordinate with professionals to refer clients to BIAC services.
• Document interactions within 72 hours of contact.
• Travel: spend about 80% of time traveling between BIAC office and courthouses.
• Attend department and organization meetings; participate in training.
• Perform other related duties as assigned.
