Brain Injury Association of Colorado, Inc. is seeking a Brain Injury Re-entry Specialist to work in Denver, CO.

This full-time hybrid role involves managing a caseload from multiple courts, screening for brain injury, and developing comprehensive re-entry and symptom support plans for survivors. You will facilitate psychoeducation groups, coordinate with professionals, and help clients access BIAC services.

Qualifications

• Bachelor’s degree, or equivalent experience, in a related field.
• Experience navigating systems in human service, case management, social work, legal systems, or other applicable settings.
• Experience with small group facilitation.
• Travel to other meetings and events within the state, sometimes after hours and on weekends.

Responsibilities

• Manage a caseload of BI survivors from 1-2 courts (Dockets TBD), 25-35 survivors.
• Screen at least 100 legal system-involved individuals for brain injury annually.
• Develop comprehensive re-entry and wraparound plans for survivors annually.
• Develop symptom support plans and wraparound service plans for survivors annually.
• Facilitate weekly psychoeducation groups for legal system-involved survivors.
• Provide wraparound services to help clients stabilize as they transition stages.
• Coordinate with professionals to share Symptom Support Plans and provide case consultation.
• Refer clients to BIAC services and track progress in systems.
