Job summary

Valley Pointe Nursing & Rehabilitation Center in Castro Valley, CA is seeking a Speech Language Pathologist for a part-time/on-call role. Compensation ranges from $62.00 to $67.00 per hour DOE.

Qualifications

• Bachelor's degree in Speech-Language Pathologist Therapy; Masters/Doctorate in Speech-Language Therapy preferred.
• Active California SLP License in good standing.
• Proficient with computer systems; experience with Point Click Care.

Responsibilities

• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Communicate with supervisor and health team regarding patient progress, problems and plans.
• Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed.
• Record evaluations, daily treatment notes, weekly progress notes, recertification documentation as required by state practice act and payers.
• Instruct families or nursing staff in maintenance program and caregiver training for discharge from therapy services.
• Participate in discharge planning.
• Comply with state practice act and facility policy.

Education

Bachelor's degree in Speech-Language Pathologist Therapy

Master's or Doctorate in Speech-Language Pathology (preferred)
