Qualifications

Responsibilities

• Assess patient physical, social, and psychological status and collaborate with the team.
• Plan an individualized care program with patient and family involvement.
• Implement and coordinate care with the interdisciplinary team while managing time and resources.
• Evaluate patient responses to interventions and adjust plans as needed.
• Promote professionalism, ethical practice, documentation and regulatory compliance.
