Zusammenfassung

Methodist Hospital is seeking a Bone Marrow Transplant Coordinator to assess needs, educate, and coordinate care for BMT patients and their families throughout the transplantation process. Travel is required, about three times per month.

The role involves coordinating pre-admission workups, reviewing clinical information, and presenting findings to the transplant physician. You will collaborate with the team to ensure seamless care and supportive services.

Qualifikationen

• Graduate of an accredited school of nursing or related healthcare degree is required.
• Minimum of three years of clinical experience in a Bone Marrow Transplant setting is preferred.
• Employee completes initial and ongoing training and competencies defined by the bone marrow service line, facility and department/unit.

Aufgaben

• Monitor patient/family before, during and after transplantation for physical, emotional, psychosocial, and financial needs and intervene as needed.
• Coordinate pre-admission work-up and visits; review pertinent clinical information and present to transplant physician.
• Collaborate with health care providers to establish care plan and coordinate services across inpatient, outpatient, and home health settings.
• Coordinate transfer of care back to referring physician and oversee long-term follow-up and post-discharge services.
• Educate patients, families, and donors about disease, processes, and post-transplant issues; act as a resource for the transplant team.
• Maintain timely communication with interdisciplinary team, patients, families, and payers for comprehensive documentation.
