Job summary

Merakey seeks a compassionate Community-Based Blended Case Manager in Warminster, PA, serving clients in Lower Bucks County. You will work directly in the community to empower individuals toward stability and independence.

You will build connections, assess needs, and develop personalized care plans, linking clients to vital resources and advocating for success. The role emphasizes culturally responsive, strengths-based support and bi-weekly case management contacts.

Qualifications

  • Requires a high school diploma plus 12 college credits in Psychology, Sociology, Social Welfare, or Gerontology and 2 years of behavioral health direct care experience.
  • Or a Bachelor’s degree with major coursework in sociology, social work, social welfare, psychology, gerontology, anthropology, or related fields.
  • Must possess a valid driver’s license and have a clean driving record; own a reliable vehicle; able to work 9am-5pm Monday-Friday; optional paid on-call available.

Responsibilities

  • Build meaningful connections while assessing client needs and creating personalized care plans.

  • Deliver culturally responsive, strengths-based support to improve quality of life.
  • Promote a rehabilitation and recovery-focused, consumer-driven approach to case management and crisis services.
  • Serve as primary case manager for assigned clients with bi-weekly support contacts in appropriate settings.
  • Develop and maintain consumer-driven service and safety plans, coordinate benefits, housing, and all aspects of service delivery.
  • Locate clients through family or community contacts when necessary.
  • Collaborate with service providers, monitor progress, and maintain accurate records of hospitalizations, crises, and incarcerations.
  • Gather and update social histories and clinical summaries annually for each client.

Responsibilities:

  • Promote a rehabilitation and recovery-focused, consumer-driven, strength-based approach to case management and crisis services.
  • Serve as primary case manager for assigned consumers, ensuring bi-weekly support contacts in appropriate settings.
  • Develop and maintain consumer-driven service and safety plans, coordinate benefits, housing, and all aspects of service delivery.
  • Locate consumers through family or community contacts when necessary.
  • Collaborate with service providers, monitor progress, and maintain accurate records of hospitalizations, crises, and incarcerations.
  • Gather and update social history and clinical summaries annually for each consumer.
  • Participate in treatment team, service plan, and provider meetings; follow through on clinical decisions using problem-solving skills.
  • Assist consumers with basic living needs, including housing, food, healthcare, transportation, socialization, education, and employment.
  • Ensure annual physicals and monitor medical health through documentation.
  • Complete reauthorization paperwork promptly and attend all meetings, supervision, and training professionally.
  • Act as liaison with families, providers, and community supports; maintain professionalism per corporate policy.
  • Complete all documentation accurately and on time; fulfill on-call duties as assigned.

This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
