Job summary

Merakey is seeking a Blended Case Manager to join our team around Monaca, PA. You will serve as the primary contact for individuals in the community, developing personalized service plans and coordinating with other providers to support goals.

The role involves counseling, helping clients transition from jail back into the community, and participating in on-call duties as needed. Requires a high school diploma or GED, 12 social science credits, and 2+ years of human services experience with 1

• Requires a high school diploma or GED and at least 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science.
• 2+ years’ experience in public or private human service, with 1 year direct care, experience must be with clients over 18.
• Valid driver’s license required.
• All OMHSAS regulations require certain clearances.

Responsibilities

• Serve as the primary contact for up to 30 individuals in the community.
• Develop, coordinate, and implement personalized service plans.
• Provide case management, service planning, and coordination with other agency providers.
• Offer supportive counseling to help individuals achieve their goals.
• Work alongside county reentry grants to support individuals transitioning from jail back into the community.
• Participate in on-call responsibilities as needed.

Earn $23.19/hour with the selection of the Enhanced Pay Option.
