Qualifications

  • High School Diploma with 12 college credits in a related field; Bachelor's preferred.
  • At least 1-year direct client care experience in behavioral health.
  • Background in Case Management preferred.

We are hiring for our Blended Case Management (BCM) Team as a Blended Case Manager, assisting children and adults with mental health challenges to access necessary supports across mental health, physical health, social, financial, vocational, and educational services. This role offers valuable experience in the mental health field. Are you still in school? You can work full-time while completing your education and receive Tuition Assistance. Our Director specializes in trauma-informed care and will provide mentorship.

Minimum Qualifications
  • High School Diploma + 12 college credits in a related field; Bachelor's preferred
  • At least 1-year direct client care experience in behavioral health; internship/volunteer considered
  • Background in Case Management preferred
  • Experience with community-based services preferred
  • Proficiency with Electronic Health Records
  • Valid Driver’s License
  • Excellent communication skills
  • Computer proficiency
  • Ability to meet physical requirements; accommodations available
  • Must be fully vaccinated or have approved accommodations
Job Responsibilities
