Job description

JOIN OUR COMMUNITY ACTION TEAM:
BLENDED CASE MANAGER/HOUSING SPECIALIST.

Are you passionate about creating meaningful change in your community? At the Sixth Street Shelter, part of Community Action Lehigh Valley, we believe everyone deserves a safe place to call home. As a Blended Case Manager/Housing Specialist, you'll be on the frontlines of helping families overcome homelessness, regain stability, and thrive.

YOUR IMPACT AS OUR BLENDED CASE MANAGER/HOUSING SPECIALIST

As a Blended Case Manager/Housing Specialist, you'll provide hands-on support to families living at the Sixth Street Shelter. Your day-to-day will include conducting intake assessments, developing individualized service plans, and providing regular case management meetings. You'll offer guidance on budgeting, employment, parenting, and other life skills, while also helping families navigate the transition to permanent housing.

This role blends direct client support with operational responsibilities, such as maintaining accurate case notes, collaborating with community partners, coordinating donations, and ensuring shelter procedures are followed. Whether leading resident meetings, advocating for services, or planning enrichment activities, you'll play a vital role in creating a safe, supportive environment where families can regain stability and independence.

WHAT YOU NEED TO BE SUCCESSFUL:

To thrive in this role, you'll need compassion, flexibility, and the ability to work as part of a collaborative, mission-driven team. A high school diploma or equivalent is required, and at least two years of experience in case management, social services, or a related field is preferred—but we also welcome applicants with equivalent experience and a strong passion for supporting families in crisis. Success in this position requires strong interpersonal and communication skills, a trauma-informed approach, and the ability to problem-solve creatively in a fast-paced environment.

You'll need to be organized, tech-comfortable, and comfortable handling data and documentation. As part of a close-knit team, we're looking for someone who's dependable, responsive, and always willing to step in to support colleagues and residents alike. A valid driver's license, personal vehicle, and flexibility to occasionally work evenings, weekends, and on-call shifts are also required.

READY TO MAKE A DIFFERENCE? If you thrive in a role where compassion meets action—we invite you to apply and join our dedicated team working every day to create pathways to housing stability, self-sufficiency, and hope.
