We are hiring for:

Blended Case Manager

Type:

Position Overview:

Blended Case Management (BCM) is a service that assists children and adults with behavioral health/medical needs by building a network of community resources. The program links clients with needed services, advocates for clients, helps negotiate systems like education and social services, and provides crisis intervention. On-call services are available 24/7 for support.

Minimum Requirements:
  • Bachelor’s degree in human services or related fields, or Associates Degree with relevant credits and 2 years of experience in mental health/human services.
  • Valid driver’s license.
