This role ensures quality care delivery, regulatory compliance, staff development, and strong collaboration with community partners to support meaningful recovery outcomes for clients.

Responsibilities

• Provide clinical and operational oversight of the Blended Case Management (BCM) program, ensuring high-quality, community-based care for individuals with serious and persistent mental illness.
• Supervise, coach, and support BCM staff through weekly clinical supervision, crisis guidance, strengths-based problem solving, and participation in hiring and onboarding.
• Lead service planning, admissions, referrals, discharge coordination, and treatment team meetings to promote continuity of care and positive client outcomes.
• Monitor documentation, productivity, and compliance to meet county, managed care, and state requirements while assisting with required program reports.
• Serve as a liaison with families, providers, and community partners, representing program operations and fostering collaborative, recovery-oriented services.

Education

Bachelor’s degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling, or education

Registered Nurse (RN)

Bachelor’s degree in nursing

The pay range for this position starts at $48,000 annually, based on experience, qualifications, and other applicable factors.
