Job summary

State of North Carolina - Health & Human Services is seeking an Early Intervention Service Coordinator (S) to provide direct service coordination for infants and toddlers birth to three, located in Lumberton and serving Robeson, Cumberland, Bladen and Sampson counties. You will collaborate with families, providers, and a multi-disciplinary team to implement IFSPs and coordinate services.

Qualifications

• Bachelor’s degree in specified fields and two years of professional experience working with the population served.
• Alternative: human services degree with three years of professional experience.
• Equivalent combination of education and experience as allowed by the vacancy announcement.

Responsibilities

• Develop functional IFSPs with families and multi-disciplinary teams.
• Coordinate and facilitate early intervention services and transition planning.
• Conduct home visits and monitor/authorize early intervention services.
• Travel to and work across the CDSA catchment area as required.

Education

Bachelor’s degree in Special Education, Elementary Education or Early Childhood Education, Psychology
