Job summary

COMHAR is seeking bilingual Targeted Case Managers to assist adults, including young adults and members of the Hispanic community, in assessing needs, developing service plans, and coordinating community supports. You will meet clients in homes, hospitals, or community settings, providing hands-on guidance and advocacy to help navigate services and achieve goals.

This full-time position in the Philadelphia area requires Spanish-English bilingual ability, a commitment to equity, and a schedule

Qualifications

• Bilingual English/Spanish preferred; strong reading and writing skills.
• High school diploma with 12 college credits in behavioral health/human services required.
• Associate's or Bachelor's in a related field preferred.

Responsibilities

• Assess clients' needs and develop service plans to promote independence.
• Maintain and review client records and service plans regularly.
• Submit documentation for all services provided.
• Participate in daily meetings and supervision sessions.
• Support clients in crisis management and de-escalation.
• Coordinate resources and transportation for treatment programs.
• Monitor attendance and advocate for needed community services.
• Maintain program productivity and client engagement.
• Meet clients in homes or in community settings to provide services.

Skills
