HealthCare Recruiters International (HCRI) is seeking a Substance Use Disorder (SUD) Case Manager for the Adolescent Youth Behavioral Health Department in San Francisco. The role covers coordination of prevention and early-intervention services, with case management, advocacy, and linkage to resources on-site in Mission District.

The position requires bilingual Spanish/English capabilities, a Bachelor’s in Social Sciences or equivalent with 2+ years in SUD services, and experience with youth

Qualifications

• Bilingual Spanish/English required.
• Bachelor’s in Social Sciences or equivalent with 2+ years in case management and SUD services.
• 2+ years supporting youth ages 12–24 and families, especially Latinx/Mayan communities.
• Experience with justice-involved or high-risk youth preferred.

Responsibilities

• Coordinate and deliver SUD prevention and early-intervention services.
• Provide case management, advocacy, and linkage to resources.
• Facilitate brief interventions and harm-reduction workshops for a small youth caseload.
• Conduct outreach and SUD screenings; provide brief interventions.
• Deliver harm-reduction workshops and coordinate treatment referrals.
• Develop and manage client case plans and provide ongoing care coordination.
• Maintain accurate, confidential documentation and reports.
• Collaborate with partner agencies and participate in required meetings/events.
