Casa Esperanza, Inc. seeks a Treatment Coordinator to provide individualized substance use counseling and case management in Boston.

You will perform intake, assessments, treatment planning, and on-call duties within a collaborative team, offering services in both Spanish and English. The role requires a strong background in motivational counseling, crisis management, and HIPAA compliance, with opportunities for professional growth and credential support.

Qualificações

• Experience with homelessness, co-occurring disorders, and people living with HIV/AIDS.
• Knowledge of substance abuse field and 12 Step philosophy.
• Ability to conduct psychosocial assessments and develop individualized treatment plans.
• Comfort coordinating admissions, referrals, and service coordination.

Responsabilidades

• Coordinate intakes, admissions and discharges.
• Complete psychosocial assessments and referrals to mental health services.
• Develop individualized treatment and service plans.
• Provide core counseling functions: screening, intake, orientation, assessment, planning, counseling, case management, referrals, education, crisis intervention, record keeping.
• Coordinate referrals to internal and external services (legal, medical, housing, employment, transportation).
• Collaborate with external providers to network services and ensure coordinated care.
• Maintain up-to-date client files and participate in team meetings.

Associate or Bachelor degree in social work/psychology/counseling

Licensed or pursuing licensure

CADAC certification within first year
