Casa Esperanza, Inc. is seeking a Treatment Coordinator to provide individual substance use counseling and case management using motivational counseling approaches. The role includes admissions coordination, treatment reporting, and data entry, with mandatory bilingual Spanish/English skills.

You will work in an integrated, holistic, team-based model across Casa programs to ensure wraparound services and clear communication with providers, clients, and external partners.

Formación

• Experience with Motivational Interviewing techniques.
• Familiarity with Cognitive Behavioral Therapy.
• Knowledge of HIPAA regulations.
• Ability to manage crises and emergencies.
• Strong written and verbal communication.

Responsabilidades

• Coordinate intakes, admissions and discharges.
• Complete psychosocial assessments and referrals as needed.
• Develop individualized treatment and service plans.
• Provide 12 core counseling functions to clients.
• Coordinate referrals to internal and external services.
• Collaborate with agencies to build a service network.
• Ensure plans are completed and client engaged in activities.
• Document encounters and prepare required reports.
• Participate in clinical team meetings and MDT.
• Manage crisis interventions and urine screening when needed.
• Develop aftercare plans with clients.

Associate or Bachelor’s degree in Social Work/Psychology/Counseling
