Hospice of the Valley is seeking a Social Worker to provide psychosocial support to patients and families, delivering counseling, discharge planning, and resource referrals. The role operates within an interdisciplinary team, primarily in the patient’s home or care facility, to support patients at critical life junctures.

The ideal candidate has a Bachelor's in Social Work with 3–5 years in palliative care or hospice, fluent Spanish, and strong assessment and communication skills.

Qualificações

• Bachelor's degree in social work; 3–5 years palliative/hospice experience.
• Fluent in Spanish; strong assessment and planning skills.
• Ability to administer psychological/emotional assessments and define treatment plans.
