Hospice of the Valley in Phoenix is seeking a hospice social worker to provide psychosocial support to patients and families within the interdisciplinary team, delivering counseling, resource referrals, discharge planning and final arrangements.

This full-time role emphasizes compassionate care, collaboration with clinical staff, and documentation, with a focus on cultural sensitivity, community resources, and supporting families through end-of-life transitions.

Qualificações

• Bachelor's degree in social work from an accredited school and a minimum of 3-5 years palliative care or hospice experience.
• Must have demonstrated ability to assess emotional needs, as well as social needs of patients and their families.
• Requires the ability to administer psychological/emotional assessment instrument and define treatment planning from results.
• Excellent communication and interpersonal skills.
• Ability to effectively collaborate with interdisciplinary team in providing patient care.
• Solution driven, creative and resourceful problem solving skills.
