Job summary

The Phoenix Rehabilitation and Nursing Center is seeking a full-time Social Worker to join our team. This role requires self-motivation, the ability to evaluate and treat independently, and a passion for geriatric care.

You will meet with residents and families to plan safe discharge, complete required forms promptly, and coordinate with other departments while arranging community referrals as needed. This is a temporary position with a competitive salary.

Qualifications

• Bachelor’s degree in Social Work or related field; LTC MDS experience preferred.
• Ability to work independently and multitask.
• Positive attitude and excellent customer service.
• Familiar with Electronic Medical Records programs; PCC preferred.

Responsibilities

• Meet with patients/residents and families to gather information and design a safe discharge plan.
• Complete all necessary forms/docs for each patient in a timely fashion.
• Collaborate with other departments to assist patients/residents.
• Make community referrals for discharged patients as indicated.
