Responsibilities

• Conduct needs assessments with families of children 0-3 and develop Family Service Plans.
• Coordinate referrals and community resources; monitor developmental status.
• Advocate for clients to receive entitled services and supports.
• Maintain client records and regulatory compliance; ensure smooth transitions to school districts.
