Job summary

Tri-Counties Regional Center in Fillmore, CA, seeks a Lead Service Coordinator II to guide a team, conduct person-centered planning, and ensure compliant service coordination for individuals with developmental disabilities. The role combines leadership, mentorship, and direct supports, emphasizing collaboration with families and community partners.

Qualifications

• Master’s or Bachelor’s degree in a Human Services or related field.
• A minimum of 2 years of social and/or mental health experience.
• Valid California driver’s license and transportation are required.

Responsibilities

• Lead Service Coordinators provide training and mentoring to staff and coordinate ongoing training.
• Review case records for Medicaid Waiver compliance and IPP/IFSPs approvals.
• Coordinate services and supports to meet individual needs and goals.
• Develop and maintain community partnerships and referral networks.
• Support team planning meetings and ensure person-centered practices are applied.
• Assist in planning conversations with individuals and families and coordinate services.
