Job summary

Far-Northern-Regional-Center in Redding, CA seeks a Service Coordinator (Bilingual Preferred English/Spanish) for a full-time position. You will provide case management to individuals with developmental disabilities, develop and monitor Individual Program Plans, and coordinate services with other agencies.

You will conduct home visits, consult with clinical staff, advocate for clients, and maintain case records.

Qualifications

• Requires equivalent to a BA degree in field related to Social Welfare or one year of experience in a like position.
• Must demonstrate fluency in English and Spanish to qualify for bilingual compensation (ability to speak, read, and write).

Responsibilities

• Provide case management services to an assigned group of persons with developmental disabilities.
• Develop and monitor Individual Program Plans.
• Coordinate services with other agencies and service providers.
• Conduct home visits with clients and families or other care providers.
• Consult with clinical and other professional staff.
• Advocate on behalf of clients.
• Complete reports and forms.
• Maintain case records.
• Other duties as assigned.
