Job summary

Qualifications

• Requires equivalent to B.A. Degree in field related to Social Welfare or one year of experience in a like position.
• Develops and monitors Individual Program Plans.
• Coordinates services with other agencies and service providers.
• Maintains case records and completes reports and forms.
