Wheeler Clinic in Manchester, CT seeks a Senior Counselor to provide comprehensive, home-based services to families involved with DCF. The role includes training others, crisis stabilization, and collaboration with schools and community providers.

The position offers a $4,000 sign-on bonus (half paid at hire, half at year one) and a bilingual requirement (Spanish/English) with a $2,288 annual stipend. The hourly rate is $23.08, and a BA/BS in a related field is required.

Qualifications

• BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field is required.
• Effective time management, strong verbal and written communication skills are required.
• Previous experience with mental health and/or co-occurring substance use disorders is preferred.

Responsibilities

• Develops case management plans or plans of care and provides direct case management services and coordination of care for assigned clients and families.
• Provides front line crisis support to individuals and families by phone, in-home, and in-community settings.
• Offers individual, group, and family counseling with supervision and coordinates treatment phases with internal and external providers.
• Maintains records of counseling, coordination of care, and team meetings for reporting and compliance with Medicaid and licensing requirements.
• Supports safety planning and crisis intervention with supervision and on-call coverage to ensure 24-hour availability.
• Participates in development and facilitation of treatment plans and transition plans with timelines and next steps.
• Meets direct service productivity goals per program guidelines and adheres to data collection protocols.
