Job summary

Hamilton-Madison House, Inc. is seeking a Senior Case Manager to direct and provide social work services at the Helen Harris Residence, a Section 2020 property serving elderly and disabled residents in New York City.

The role emphasizes care planning, interdisciplinary collaboration, and ongoing assessment with clients and their families. The position requires bilingual English and Chinese proficiency, a Human Services degree (MSW preferred), and prior experience with older adults.

Qualifications

• Bachelor's degree in a Human Services field required; Master's degree preferred in social work, counseling, psychology, gerontology, or related discipline
• Bilingual proficiency in English and Chinese required
• Three years of experience working with older adults preferred
• Sensitivity to and understanding of the needs of older adults
• Ability to handle complex and difficult situations calmly and professionally
• Ability to develop collaborative relationships with community groups and stakeholders
• Excellent computer skills including data management systems and willingness to learn additional programs

Responsibilities

• Case management: information and referral, advocacy, benefits assistance, care plans, assess and refer for mental health issues
• Service coordination: coordinate with health professionals and various agencies, annual housing recertifications
• Administration: HUD compliance, trainings, reports, coordination with management and staff
• Other duties: assist with senior service events and participate in supervision and meetings
