Job summary

Hamilton-Madison House seeks a Senior Case Manager to direct social work services at the Helen Harris Residence, a HUD-supported facility for seniors and people with disabilities in New York City.

You will assess needs, develop individualized care plans, and coordinate services with health professionals and community agencies, while supporting clients in English and Chinese. The role includes compliance tasks, supervision, and participation in staff meetings.

Qualifications

• Bachelor's degree in a Human Services field required; Master's degree preferred in social work, counseling, psychology, gerontology, or a closely related discipline.
• Bilingual proficiency in English and Chinese required
• Three years of experience working with older adults preferred
• Sensitivity to and understanding of the needs of older adults
• Ability to handle complex and difficult situations professionally
• Strong collaboration with community groups and excellent organizational skills

Responsibilities

• Provide information and referral services in English and Chinese.
• Offer advocacy and supportive contact to clients.
• Assist with benefits and entitlement applications.
• Develop and maintain individualized care plans.
• Assess and refer for mental health issues, dementia, and related conditions.
• Coordinate services with health professionals as needed.
• Provide annual housing recertifications.
• Prepare reports and vouchers; work with management and building staff.
• Participate in supervision and staff coordination meetings.
