Job summary

Asian Health Services in California’s Bay Area leads the SpringLight Health PACE program and seeks an Activity Coordinator to plan and oversee a therapeutic recreation program for PACE Participants.

You will assess needs, coordinate group and individual activities, collaborate with the IDT, and ensure safety and engagement in a culturally competent, multilingual environment, including telehealth and on-site visits.

Qualifications

• Bilingual Chinese required (Cantonese or Mandarin)
• Two years in social recreational or educational programs in the past five years, with at least one year full-time in a healthcare/elderly setting
• Bachelor’s degree in a related human services field
• Ability to conduct home visits as assigned
• Knowledge of social activities for older adults
• At least 1 year of experience with frail or elderly population
• Certification by NCTR or California RTC
• Strong understanding of aging psychosocial dynamics
• Current BLS and First Aid required

Responsibilities

• Coordinate day center social programming and activities
• Conduct assessments and participate in IDT care planning
• Ensure safe environment and mitigate risk factors
• Document progress in the electronic health record
• Develop and oversee daily Recreation Program and community outings
• Collaborate with Recreational Therapist and community resources
• Provide clear communication with Participants and caregivers
• Arrange performances, holidays, instructors and classes within budget
• Provide care in-person, via telehealth, or by phone
• Utilize Epic for workflow optimization
• Perform other duties as required
