Qualifications

Responsibilities

• Provide onsite face-to-face coaching and support.
• Identify health education needs through assessments.
• Refer clients to clinical programs as appropriate.
• Collaborate to set health improvement plans and goals.
• Lead wellness activities and group coaching sessions.
• Assist with biometric screenings and data collection.
• Support site events and coordinate with matrix partners.
