Job summary

The Registered Nurse Case Manager at MedWatch LLC will manage a full caseload, coordinating care transitions and discharge planning across providers, payers, and family. You will assess patient needs, document actions, and ensure medical necessity and appropriate benefits are in place.

This role requires an active RN license in the United States, 7 years of clinical experience, and fluency in English and Spanish.

Qualifications

• Active RN license in the United States or its territories.
• 7 years of varied clinical experience preferred.
• Good organizational skills and time management.
• Excellent verbal and written communication skills.
• Ability to speak English and Spanish fluently.
• Must have strong computer skills and MS Office proficiency.
• URAC-recognized Case Management certification within 3 years of hire.

Responsibilities

• Manage a full caseload and coordinate care transitions and discharge planning.
• Collaborate with providers, payers, and family to meet medical necessity and benefits.
• Document case actions and maintain case management records.
• Assess problems, set goals, and determine actions to meet patient needs.
• Maintain ongoing communication with the health care team and the patient.
