Behavioral Health Network (BHN) is seeking a Nurse Care Manager to join its CBHC Care Management team in Western Massachusetts. You will advocate for individuals, secure services, and monitor progress to ensure quality care in a community-based setting.

Qualifications

• RN license required.
• 1–2 years' work experience in behavioral health, human services, and/or medical field.
• Demonstrates cultural and linguistic competence with diverse populations.

Responsibilities

• Advocate for individuals served and promote autonomy.
• Coordinate services and monitor progress to ensure quality care.
• Maintain accurate and timely documentation of interactions and referrals.
• Support transitions from inpatient settings to the community.
• Perform monthly face-to-face outreach visits per caseload.
