Adelante Healthcare in Phoenix, AZ is seeking a Registered Dietitian to join the Integrated Care Team. You will assess patients’ nutritional status, develop care plans, and collaborate with clinicians to support health outcomes across the lifespan.

The role emphasizes patient-centered nutrition care, SMG development, and coordination with health education and nursing staff in a primary care setting.

Qualifications

• Four-year degree in Dietetics or related field.
• Must hold RD/RDN credentials (Registered Dietitian).
• Bilingual English/Spanish strongly preferred.
• Travel to multiple Adelante locations may be requested.

Responsibilities

• Assess nutritional status and develop evidence-based nutrition care plans.
• Collaborate with Integrated Care Team to improve patient outcomes.
• Provide nutrition education and health promotion to clients and programs.
• Document care plans and progress in the electronic health record.
• Develop Self-Management Goals (SMGs) with patients and follow up as needed.
• Deliver trainings and support to Health Education staff.
