Job summary

Adelante Healthcare seeks a Clinical Registered Dietitian I to assess nutritional status and develop evidence-based care plans for patients across the lifespan, including diabetes, hyperlipidemia, hypertension, and weight management. You will collaborate with an integrated care team to set SMART goals and coordinate follow-up appointments.

The role emphasizes patient-centered, team-based primary care within Adelante’s medical home model, with opportunities for telehealth and community outreach

Qualifications

• Master’s degree in Dietetics or related field (required for new graduates) or Bachelor's degree in Dietetics.
• Registered Dietitian (RD/RDN) credential with the Commission on Dietetic Registration.
• CPR and AED certification per AHA/Red Cross guidelines.
• Arizona Level One Fingerprint Clearance Card from the state DPS.
• Experience in community or clinical nutrition education (0–2 years for new grads).
• Current driver's license and auto insurance; ability to travel between locations.

Responsibilities

• Perform nutrition assessments using the Nutrition Care Process and document care plans.
• Collaborate with the Integrated care team to coordinate patient care.
• Educate patients on medications, supplements, and drug-nutrient interactions.
• Develop patient education materials and clinician guidelines as needed.
• Provide telehealth education and participate in trainings for professional development.
