The Clinical Registered Dietitian has the excellent opportunity to work collaboratively with the Integrated Care Team and will be responsible for assessing nutritional status and developing evidence-based nutrition care plans for patients across the lifespan with various acute and chronic conditions.As a member of the Integrated Care Team, the dietitian will work collaboratively with behavioral health consultants, registered nurses, integrated care coordinators, medical assistants, and primary care providers to improve patient health outcomes and organizational goals. The integrated dietitian will offer a stepped care approach to provide concise, action-oriented consults with patients resulting in ongoing support and guidance. The integrated dietitian will also be responsible for collaborating with patients to establish Self‑Management Goals (SMGs) and conducting follow‑ups based on PCP visit schedules or patient needs. This position is essential to Adelante’s team-based primary care medical home model of care that supports coordinated, patient-centered, comprehensive health care to all.
