Marin Community Clinics is seeking a Referral Navigator to manage and close medical and dental referrals for patients, coordinating urgent cases and advocating for patients as needed. You will support providers and patients with referral processing, authorizations, and scheduling across specialties.

Candidate must be bilingual (English-Spanish), detail-oriented with strong analytical and communication skills, and comfortable handling sensitive information and interactions with multiple

Qualificações

• Bilingual English-Spanish required and strong communication skills.
• Experience with data elements and basic quality measure reporting.
• Ability to work with patients and external organizations by phone and in person.
• Excellent written and verbal communication; detail oriented and analytical.

Responsabilidades

• Monitors and schedules patients through our Oral Surgery program.
• Maintains patient confidentiality per policy and HIPAA guidelines.
• Attends regular Referral Department and All Staff meetings as scheduled by the Manager.
• Responds to communications from clinic staff and outside facilities.
• Completes applications for Operation Access, Lions Eye Foundation and others.
• Requests prior authorizations for specialty services with various payors.
• Tracks and follows up on status of authorizations, appointments and referrals.
• Processes referrals, follows up and closes referrals; schedules specialist visits.
• Assists patients by scheduling and explaining referrals, services and resources.
• Faxes or emails medical records and insurance eligibility to specialist offices.
• Closes referrals by obtaining medical records from providers.
• Maintains professional relationships, adheres to protocols and provides timely patient information.
• Other duties as assigned.
