Job summary

Family Health Centers of San Diego is seeking a Prenatal Social Worker who provides psychosocial assessments, counseling, and referrals for pregnant patients, as part of a multi-disciplinary community health team. Requirements include a Bachelor's Degree in Social Work, Sociology or Psychology; bilingual English/Spanish; and about 2 years in community health care.

Travel between sites is occasionally required; a valid California driver's license and proof of auto insurance are recommended.

Qualifications

• Bachelor's Degree in Social Work, Sociology or Psychology required.
• Bilingual in English/Spanish required.
• 2 years of experience in community health care preferred.
• Traveling between sites and other locations is occasionally required.
• Must have a valid California driver’s license, an automobile, and proof of minimum levels of car insurance as required under California law.

Responsibilities

• Conducts psychosocial assessments and determine when clients need psychosocial interventions; provides reassessments.
• Maintains a list of current community emergency resources for each site/program.
• Partners with community resources and assists clients with external resources as needed.
• Provides psychosocial interventions.
• Facilitate individual, family, and group counseling and therapy.
• Provides referrals as appropriate.
• Promotes internal referrals to FHCSD services.
• Works as a member of the multi-disciplinary healthcare team to create proactive solutions and comprehensive treatment plans.
