Job summary

Family Health Centers of San Diego seeks a Nutritionist-Health Educator to provide nutrition assessments, education, and care plans for patients. You will create culturally appropriate curricula, plan programs, and deliver education while evaluating outcomes to promote community health.

The role requires 3 years of nutrition/education experience, a Bachelor's in related field, and bilingual Spanish/English communication. Travel between sites is occasional; valid CA driver’s license is required.

Qualifications

• 3 years of experience in nutrition and/or chronic disease education or related field.
• Demonstrated ability to translate nutrition science into patient education materials.

Responsibilities

• Conduct nutrition/health education assessments and create individual care plans.
• Provide group classes on nutrition/health topics in culturally appropriate ways.
• Document all activities accurately in the EMR according to department standards.
• Coordinate scheduling and follow-ups with patients and care teams.
