Job summary

Qualifications

• Valid RN license.

• Bilingual (English/Spanish).

Responsibilities

• Provide telephonic triage and patient guidance using clinical decision-making software.
• Track and document call center interactions, referrals, and case-managed patients.
• Support program evaluations and provide reports to improve quality and efficiency.
• Foster clear communication among the center, physicians, and statewide offices.
• Manage tasks with goals and timelines, prioritization, and collaboration.
• Operate in a flexible hybrid setting with some remote work.
