Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to perform telephonic consultations with patients using specialty medications and manage disease states. You will reconcile medications, screen for interactions, and provide side effect management while expediting orders and meeting production metrics.

The role emphasizes patient-centered care, collaboration with prescribers, and adherence to quality standards.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree.
• Current, unrestricted pharmacist license in state of residence; must continually obtain education credits to maintain active licensure.
• At least two years of experience as a practicing licensed Pharmacist preferred.
• Strong clinical background.
• Experience with Specialty Pharmacy medications desirable.
• Participate in, adhere to, and support compliance program objectives.
• Ability to interact cooperatively and respectfully with other employees.

Responsibilities

• Provide telephonic patient education for specialty pharmacy patients focusing on disease state management, therapy, storage, and administration.
• Document clinical consultations and interventions in the pharmacy software per policies and URAC standards.
• Interpret and evaluate prescriptions; contact providers/patients to verify details and expedite orders; verify controlled substances.
• Coordinate with physicians for new prescriptions and transfers; discuss generic/biosimilar options where possible.
• Verify prescription data in the system and answer clinical questions from the pharmacy team.
• Adapt to new workflows; stay current with specialty therapies and new approvals.
• Back up other pharmacists and train less experienced staff as needed.
• Report medication errors and participate in quality improvement activities.
• Operate in compliance with federal/state laws and ethical standards.
• Other duties as assigned.
