Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to join our team performing telephonic consultations with patients on specialty medications, medication reconciliation, and management of disease states. The pharmacist will interact with prescribers, expedite orders, and maintain quality standards.

The role requires a PharmD or BS Pharmacy and an active state license, with 2+ years of experience, strong clinical background, and knowledge of specialty drugs.

Qualifications

• Graduate of an ACPE accredited school with a PharmD or BS Pharmacy degree.
• Current, unrestricted pharmacist license in home state; active licensure required.
• At least two years of licensed pharmacist experience preferred.
• Strong clinical background; familiarity with specialty medications desirable.

Responsibilities

• Provide telephonic patient consultations for specialty medications and disease state management.
• Perform medication reconciliation, screen for interactions, and offer side effect management advice.
• Interpret prescriptions, contact prescribers for clarification, and expedite orders.
• Document clinical interventions and maintain production metrics and service levels.
• Back up and train junior pharmacists as needed; comply with ISMP/quality standards.
