Lumicera is seeking a Pharmacist, Clinical Consultation to join our team. Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree.
• Current, unrestricted pharmacist license in state of residence; must continually obtain education credits to maintain active licensure.
• At least two years of experience as a practicing licensed Pharmacist preferred.
• Strong clinical background required.
• Experience and/or strong understanding of Specialty Pharmacy medications desirable.
• Participate in, adhere to, and support compliance program objectives.
• The ability to consistently interact cooperatively and respectfully with other employees.

Responsibilities

• Provide patient education through telephonic consultations for specialty pharmacy patients, focusing on disease state management and medication administration.
• Document consultations and interventions in the pharmacy software per internal policies and external regulations (URAC).
• Interpret and evaluate prescriptions; verify details with physicians/patients to expedite processing; verify controlled substances.
• Contact physicians for new prescriptions and discuss pharmaceutical questions; work with physicians to optimize drug choices.
• Verify system-entered prescription information and answer questions from the pharmacy team.
• Stay current with advances in specialty therapies and guidelines; adapt to new workflows and processes.
• Back up other pharmacists as needed and train less experienced staff.
• Document and report medication errors; support quality initiatives and ISMP standards.
• Operate within all applicable federal and state laws and uphold high ethical standards.
