Job summary

The Arc of Opportunity in North Central Massachusetts is seeking a Person-Centered Plan Manager (Shared Living) to support individuals with developmental disabilities. This full-time role involves monthly home visits, documentation, and coordination with care providers to ensure comprehensive, compliant care.

Hours are typically Monday through Friday, 8am to 4pm, with flexibility to meet program needs. The employer offers medical, dental, vision coverage, paid time off, and retirement options.

Qualifications

• Must pass background checks and pre-employment screenings including TB test and CORI.
• Bachelor's degree in social work, human services, or a related field (or equivalent experience).
• Basic computer skills using Microsoft Office, email, internet, and documentation entry.
• Strong interpersonal and communication skills to collaborate with program participants, their Care Providers, and a multidisciplinary team.
• Ability to prioritize tasks, work independently and within a team, and meet deadlines.
• Compassionate, resourceful, and eager to learn.
• Valid driver's license and reliable personal vehicle meeting state requirements.

Responsibilities

• Conduct monthly home visits for program members and their Care Providers, ensuring well-being and regulatory compliance.
• Complete all necessary documentation accurately and promptly to maintain thorough records of each visit.
• Maintain open lines of communication with assigned staff and coordinate with other service providers to ensure comprehensive care.
• Make appropriate referrals to external resources when additional support is needed.
• Monitor the home setting according to regulatory guidelines to ensure safety and comfort for individuals with disabilities.

Salary: $24.95-$25.95 Per Hour

Hiring Bonus-$2500

Schedule

Monday-Friday 8am-4pm (40 hr/week) *flexibility with schedule needed to meet programs needs*

Essential Job Functions
