Job summary

THERAPY 2000 is seeking a licensed Pediatric Physical Therapist to join a mission-driven team serving Texas families with home-based services. You’ll perform PT evaluations, develop treatment plans, and create home exercise programs, coordinating with caregivers and other providers to ensure seamless care.

You bring a Texas PT license, a collaborative spirit, reliability, and flexibility to adapt to patient needs. Prior home-health experience and bilingual skills are a plus.

Qualifications

• Current/active Texas PT license.
• Innovative spirit with reliability.
• Flexibility to shift resources and priorities as needed.
• Ability to use common workplace technology (laptop, email, web browser).

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Coordinate care with other providers and disciplines.
• Complete minimum weekly patient visits as required by the agency.
• Meet deadlines for chart updates, progress summaries, call logs, and paperwork.
• Adhere to agency policies, procedures, and relevant laws for home-health pediatric PT.
