Job summary

Humboldt Park Health in Chicago, IL is seeking a Part Time Bilingual Community Health Worker (Pediatric Van) to serve as a resource for participants, perform home visits, and develop community health referrals. Qualifications

• Completion of Community Health Worker program required.
• Language requirement: bilingual in English and Spanish (read/write).

Responsibilities

• Serve as a resource for participants in the program.
• Develop referral relationships with healthcare providers, social workers and other community based service providers.
• Perform home visits for education, support, home health assessments, obtaining and completing consents.
• Follow-up on provider referrals and assisting families in addressing barriers to health care access.
• Strong organizational and planning skills.
• Provides outreach to the community.
• Assist in the development of resource materials and preparation and maintenance of accurate records and data.
• Complete all reports to document activities and provide data to measure impact.
• Performs other administrative office duties.
• Maintains department orderliness and cleanliness.
• Other duties as assigned.
