Job summary

CHW Solutions, LLC seeks two part-time Community Health Workers to deliver six months of home-visiting services to families with preschoolers in the Twin Cities metro area, as part of a CDC-funded research project with the University of Minnesota. Responsibilities include six monthly visits, goal setting, Motivational Interviewing, documenting in the EHR, and coordinating with clinics.

The roles require bilingual skills (Spanish and/or Somali and English), a valid driver’s license, reliable

Qualifications

• Excellent communication skills with patients and partners.
• Experience delivering health education services.
• Fluency in Spanish and/or Somali and English.
• Driver’s license and reliable vehicle.

Responsibilities

• Deliver six monthly home visits to families with preschool children.
• Set goals and implement routines with families.
• Use Motivational Interviewing and coaching methods.
• Engage families in activities supporting child development.
• Document visits in the EHR and input research data.
• Coordinate with University of Minnesota staff and CHW leadership.
• Attend biweekly team meetings and ensure protocol adherence.
