Job summary

Health Connect America is seeking a Therapist I to provide high-quality clinical services to children, families, and adults in our Minnesota-based program. You will conduct assessments, develop and monitor treatment plans, document progress, and collaborate with a multidisciplinary team to promote evidence-informed care in home, office, and community settings.

Excellent communication, empathy, and a commitment to client-centered care are essential as you engage in outreach to expand referrals

Qualifications

• Master's degree in Social Work, Counseling, or related field is required.
• Minimum of one year experience working with children and/or families.

Responsibilities

• Provide high-quality therapeutic services to clients across settings.
• Conduct assessments and develop individualized treatment plans.
• Maintain accurate documentation and collaborate with multidisciplinary teams.
• Engage in outreach and marketing efforts to promote services and attract referrals.
