Job summary

Qualifications

• Bilingual (Spanish), both oral and written proficiency required.
• Analytical skills in evaluating patients to plan and provide navigation.
• Ability to communicate effectively with diverse audiences including staff, patients, community, media, and agencies.
• Interpersonal skills to allay anxiety, elicit cooperation, and educate patients and families.
• Strong organizational skills to maintain files, records, and data.
• Proficient typing skills for preparing materials.

Responsibilities

• Assists in coordinating referral appointments with health professionals (genetic counselor, nutritionist, psychiatrist, social workers, etc.).
• Assists patients in gaining access to support services including financial assistance with medications and co-pays, transportation to treatment, and support programs.
• Triages for needed services across all product or service lines.
• Accompanies patients to medical visits and coordinates follow-up care.
• Provides support within effective and realistic time frames.
• Assists with coordinating patient and family education and counseling regarding disease and treatment.
• Enhances patients’ satisfaction by responding to their support needs during illness.
• Participates in ongoing quality assurance activities.
• Collects and reports information to ensure grant compliance and reporting requirements.
• Participates in ongoing education and networking regarding navigation for patients with language barriers.
• Participates in outreach programs that target underserved patients.
• Speaks publicly, in Spanish, at outreach programs serving at-risk populations.
